Chafee Foster Care Independence Program Services (CFCIP)
Family Facets provides services to ensure that every young adult that has transitioned from foster care has the opportunity to access education, housing, emergency services, employment, health care, community services and that these young adults have a connection with a caring adult and/or life long family member. The CFCIP services are tailored to each transitioning young adult, based on the specific strengths and needs of each young adult. CFCIP is designed to complement the youth’s own efforts to achieve self-sufficiency and to assure that program participants recognize and accept their personal responsibility in preparing for and making the transition from adolescence to adulthood. Each young person will come to the Children's Division with unique strengths, different experiences and goals. The CFCIP services are intended to help these youth, ages 14-21, choose the path they want to follow, and then help them achieve their goals.

Counties Served in the Northeast Region CFCIP Site
The Northeast Region for CFCIP program includes the counties of Schuyler, Scotland, Clark, Adair, Knox, Lewis, Macon, Shelby, Marion, Monroe, Ralls, Randolph, Howard, Boone, Callaway, Audrain, Warren, Montgomery, Lincoln, Cole, Osage, Gasconade, Franklin, Jefferson, St. Charles, Pike
Individualized Areas of CFCIP Services
Family Facets provides individualized CFCIP services in the general areas of Academic Achievement, Job Readiness, Community Services and Support, Youth Leadership, and Independent Living Skills, as well as services custom tailored to each youth's specific needs.
Referral Assessments
Each transitioning young adult comes to the Family Facets CFCIP program with an Adolescent Family Support Team (FST) Guide & Individualized Action Plan (IAP), and the most recent Ansell-Casey Life Skills Assessment (ACLSA) as provided by the Children's Division.
Referrals
Children's Division staff may make Chafee Foster Care Independence Program Services referrals of transitioning young adults by sending referral information to the Children's Division Regional Older Youth Transition Specialist.
