Chafee Foster Care Independence Program Services

Family Facets provides services to ensure that every young adult that has transitioned from foster care has the opportunity to access education, housing, emergency services, employment, health care, community services and that these young adults have a connection with a caring adult and/or life long family member.  The CFCIP services are tailored to each transitioning young adult, based on the specific strengths and needs of each young adult.

Referrals

Children's Division staff may make Chafee Foster Care Independence Program services referrals of transitioning young adults by sending referral information to the Children's Division Regional Older Youth Transition Specialist.
